Responsibilities

• Conduct comprehensive physical therapy evaluations for residents in the skilled nursing setting and establish individualized treatment plans
• Implement evidence-based therapies to improve mobility, strength, balance, and functional independence
• Reassess resident progress regularly; adjust treatment plans and goals accordingly
• Document care in accordance with regulatory requirements and facility policies; communicate progress to the interdisciplinary team
• Collaborate with nursing, therapy, and other care colleagues; participate in discharge planning and care transitions
  
  

Requirements

• Active Maryland Physical Therapy license
• Doctor of Physical Therapy (DPT) degree from an accredited program
• Experience in skilled nursing facility or geriatric rehabilitation settings preferred
• Strong communication and documentation skills; ability to work within a multidisciplinary team
• Ability to travel and adapt to a temporary assignment environment
