Responsibilities

• Conduct initial physical therapy evaluations and ongoing reassessments for residents in a long-term care setting
• Develop and implement individualized treatment plans to improve mobility, strength, balance, and functional independence
• Deliver evidence-based PT interventions within scope and facility policies; monitor and document progress; adjust Plan of Care as needed
• Document evaluations, goals, progress notes, and discharge plans in patient records, ensuring accuracy and regulatory compliance
• Collaborate with an interdisciplinary team (nurses, therapists, physicians) to coordinate care and support transitions of care
• Educate residents and families on home exercise programs, safety strategies, fall prevention, and activity modification
  
  

Requirements

• Active physical therapy license to practice in Maryland (or eligibility to obtain licensure)
• Ability to work a 38-hour week on a 08:00–16:30 schedule in a long-term care setting
• Willingness to travel for temporary assignment and-support a 13-week contract
• Strong communication, documentation, and teamwork skills to coordinate with the care team
• Demonstrated ability to work in a regulated healthcare environment and adhere to facility policies
