Responsibilities

• Provide one-on-one physical therapy in patients’ home settings, focusing on goals and independence
• Assess condition and develop individualized plans of care to maximize mobility and functional outcomes
• Collaborate with an interdisciplinary team to ensure patient needs are met
• Conduct standard therapy measurements (ROM, strength, balance) and apply a range of modalities
• Perform OASIS-based documentation and case management; maintain accurate EMR records
  
  

Requirements

• Active Vermont physical therapy licensure
• Home health experience with OASIS documentation required
• EMR experience preferred (McKesson/Netsmart strongly preferred)
• Ability to travel to patients’ homes within the service area
• Availability for the full 13-week assignment with no planned time off
