Responsibilities
- Provide one-on-one physical therapy in patients’ home settings, focusing on goals and independence
- Assess condition and develop individualized plans of care to maximize mobility and functional outcomes
- Collaborate with an interdisciplinary team to ensure patient needs are met
- Conduct standard therapy measurements (ROM, strength, balance) and apply a range of modalities
- Perform OASIS-based documentation and case management; maintain accurate EMR records
Requirements
- Active Vermont physical therapy licensure
- Home health experience with OASIS documentation required
- EMR experience preferred (McKesson/Netsmart strongly preferred)
- Ability to travel to patients’ homes within the service area
- Availability for the full 13-week assignment with no planned time off
