Responsibilities
- Provide skilled physical therapy assistant services under supervision within a LTAC/LTC setting
- Assist with develop and implement treatment plans in collaboration with the supervising PT and rehab team
- Support therapeutic exercises, gait and mobility training, range of motion, and functional training
- Document patient progress and update charts; communicate changes to the supervising PT and interdisciplinary care team
- Maintain patient safety, proper body mechanics, and infection control; support safe transfers and daily care activities
- Educate patients and families on home exercise programs, fall prevention, and safety strategies

Requirements
- Active Physical Therapy Assistant license in New Hampshire or eligibility for licensure
- Graduation from an accredited PTA program
- Ability to work in a long-term care setting and participate in a travel assignment
- Strong communication and teamwork skills; proficient documentation and record-keeping
- Comfort coordinating with a multidisciplinary rehab team and adhering to standard care protocols
