Responsibilities

• Administer prescribed medications and treatments per the care plan and physician orders
• Monitor residents’ conditions, vital signs, and report changes promptly to the supervising RN
• Provide routine nursing care and assist residents with activities of daily living
• Document assessments, interventions, and outcomes accurately in the resident chart
• Assist with wound care, routine procedures, and specimen collection as directed
• Collaborate with the interdisciplinary team to ensure safe, person-centered care and regulatory compliance
  
  

Requirements

• Active Licensed Practical Nurse license in the state of New Hampshire
• Experience in long-term care or skilled nursing settings preferred
• Ability to work the listed schedule (7:00–15:30) and perform on-site in Bedford, NH
• Strong communication, documentation, and teamwork skills
• CPR/BLS certification preferred or required by assignment terms
