Responsibilities

• Provide direct nursing care to residents within the scope of an LPN in a long-term care setting
• Administer prescribed medications and monitor for adverse effects, documenting accurately per policy
• Conduct routine assessments, monitor vital signs, and help update care plans as needed
• Collaborate with registered nurses and other members of the healthcare team; promptly report changes in condition
• Assist residents with activities of daily living while promoting safety, dignity, and independence
• Maintain infection control practices, safety standards, and complete accurate charting and documentation

• Active LPN license
• Ability to work evenings and 8-hour shifts
• Experience in or comfort with long-term care or skilled nursing settings preferred
