Responsibilities

• Administer medications and monitor residents’ conditions; document care accurately
• Perform routine assessments and report changes to the supervising nurse or physician
• Assist residents with activities of daily living (bathing, dressing, mobility)
• Provide wound care, comfort measures, and routine clinical procedures as delegated
• Maintain safety and infection control protocols; supervise fall prevention and patient transfers
• Collaborate with the interdisciplinary team to update and implement individualized care plans
  
  

Requirements

• Active Licensed Practical Nurse license in New Hampshire
• Graduation from an approved practical nursing program
• Prior long-term care or skilled nursing experience preferred
• Ability to work evenings (3:00 PM–11:30 PM) and every other weekend
• Strong clinical assessment, documentation, and communication skills
• Ability to thrive in a fast-paced LTC environment and work collaboratively with the team
