Greenfield Rehabilitation Agency is seeking a motivated and highly engaged Part-Time Senior Community Coordinator (PT or OT) to support our senior living communities and skilled nursing facilities in Delavan, WI and the surrounding areas.
This unique role blends direct patient care, team engagement, and community relationship-building, making it the perfect fit for a therapist who enjoys both hands-on clinical practice and leadership influence within the senior care setting—on a part-time basis.
Position Overview
In this role, you will:
- Provide individualized therapy services to aging adults within assigned senior communities
- Support, mentor, and collaborate with therapy teams across multiple sites
- Serve as a primary contact for facility administrators and community partners
- Help strengthen relationships and promote Greenfield Rehab’s mission and values
- Work closely with the Regional Director to ensure clinical excellence, team satisfaction, and smooth daily operations
This is a part-time hybrid clinical and leadership opportunity ideal for a PT or OT who enjoys autonomy, communication, and building strong care environments for seniors.
Qualifications
- Licensed Physical Therapist (PT) or Occupational Therapist (OT) in the State of Wisconsin
- Experience in senior care settings (ALF, SNF, Memory Care, or Home Health) strongly preferred
- Strong communication, leadership, and organizational skills
- Ability to support multiple communities and foster positive relationships
- Minimum 1 year of experience preferred, though strong candidates with less experience will be considered
