Physical Therapist Responsibilities
-
Perform initial and ongoing evaluations of residents’ physical abilities, mobility, strength, balance, and functional status
-
Identify impairments and barriers to safe and independent mobility
-
Develop and implement individualized, goal-driven physical therapy treatment plans
-
Address gait, balance, endurance, and functional mobility needs
-
Support recovery following injury, surgery, illness, or neurological conditions
-
Treat residents with chronic conditions such as arthritis, Parkinson’s disease, osteoporosis, or post-stroke deficits
-
Improve performance in functional activities including transfers, walking, stair negotiation, and fall recovery
-
Provide therapeutic exercise, neuromuscular re-education, and manual therapy as appropriate
-
Recommend, fit, and educate residents on assistive devices and durable medical equipment
-
Perform fall risk assessments and implement fall prevention strategies
-
Educate residents, families, and staff on safe mobility techniques and home safety
-
Collaborate with interdisciplinary teams including OT, SLP, nursing, and care staff
-
Participate in care planning meetings to support holistic resident outcomes
-
Maintain accurate, timely documentation and ensure regulatory compliance

Qualifications:- Graduate of an accredited school with a minimum of a bachelor’s degree in Physical Therapy.
- Certification or license as required by the State of Wisconsin or applicable state to practice as a Physical Therapist.
