Greenfield Rehabilitation Agency is seeking a motivated and highly engaged Senior Community Coordinator (PT or OT) to support our senior living communities and skilled nursing facilities in Delavan, WI and the surrounding areas.
This unique role blends direct patient care, team engagement, and community relationship-building, making it the perfect fit for a therapist who enjoys both hands-on clinical practice and leadership influence within the senior care setting.
Position Overview
In this role, you will:

Provide individualized therapy services to aging adults within assigned senior communities

Support, mentor, and collaborate with therapy teams across multiple sites

Serve as a primary contact for facility administrators and community partners
-
Help strengthen relationships and promote Greenfield Rehab’s mission and values
-
Work closely with the Regional Director to ensure clinical excellence, team satisfaction, and smooth daily operations
This is a hybrid clinical and leadership opportunity ideal for a PT or OT who enjoys autonomy, communication, and building strong care environments for seniors.
Qualifications

Licensed Physical Therapist (PT) or Occupational Therapist (OT) in the State of Wisconsin
-
Experience in senior care settings (ALF, SNF, Memory Care, or Home Health) strongly preferred
-
Strong communication, leadership, and organizational skills
-
Ability to support multiple communities and foster positive relationships

Minimum 1 years of experience preferred, though strong candidates with less experience will be considered

Qualifications:- Graduate of an accredited school with a minimum of a bachelor’s degree in Physical Therapy or Occupational Therapy
- Certification or license as required by the State of Wisconsin or applicable state to practice as a Physical Therapist or Occupational Theraist
- At least 1 years clinical experience
