CNA - Certified Nursing Assistant

Job Details: SIGN ON BONUS!

• Must have a current state of MI, CNA license
• Must have a valid Driver's license and auto insurance, and kept current at all times
• Expected to rotate weekends and holidays
• Will be expected to work in the day center and/or as a home health aide in participant homes throughout our "service area" (map of area can be provided)
• Working hours will vary for CareShare (homecare) and day center, depending on participants needs
• Will consider Part-time status for CareShare.

Location:

• Saginaw- (Day center location)
• Bay City
• Midland and surrounding service areas
• **Please note for our CareShare Program: you will be driving to participants homes and helping care for participants in their homes. You will also be working in our Day Center, located in Saginaw township.

Reports to:

• Day center supervisor, CareShare Supervisor, Center Manager and/or designee

Some Basic Job Duties:

• Responsible for the daily direct care of PACE participants; providing care to our participants in their homes for our CareShare Program and/or Day Center.
• Provides basic ADL care to assigned participants.
• Communicates effectively with participants, families, staff and staff of other agencies.
• Participates in Interdisciplinary Team meetings
• Documents treatments/procedures, observations, nursing care and participant activities on appropriate forms

Requirements:

• Current CNA certificate required
• At least one year experience working with a frail elderly population is required
• Current CPR certification required, if needed we will provide training
• Positive, interpersonal skills with a clear understanding of confidentiality and HIPAA principles.

Job Types: Full-time, Part-time
