Essential Responsibilities
- Provide comprehensive primary care services including preventive care, chronic disease management, and acute care
- Perform patient assessments, develop treatment plans, and prescribe medications
- Coordinate patient care with specialists and healthcare team members
- Maintain detailed electronic medical records and documentation
- Lead patient education initiatives and preventive care programs
- Participate in quality improvement initiatives and population health management
- Supervise clinical support staff and contribute to team development
- Master's degree in Nursing (MSN) from an accredited institution
- Current New York State RN and Nurse Practitioner licenses
- Board Certification in Internal Medicine
- Current BLS certification
- Proven experience in outpatient primary care
