- Deliver comprehensive primary care services including preventive care, acute treatment, and chronic disease management
- Conduct patient assessments, develop treatment plans, and prescribe medications
- Coordinate patient care with specialists and healthcare teams
- Maintain detailed EMR documentation and ensure regulatory compliance
- Lead clinical staff and contribute to quality improvement initiatives
- Participate in population health management programs
- Build and maintain a sustainable patient panel
- Current New York State RN and Nurse Practitioner licenses
- BSN and MSN from accredited institutions
- Board Certification as a Family Nurse Practitioner
- Current BLS certification
- Strong clinical judgment and diagnostic abilities
- Excellent communication and interpersonal skills
