What are the responsibilities and job description for the QUALITY SUPPORT SPECIALIST position at Graves Gilbert Clinic?
A Quality Support Specialist is a team member who facilitates patient continuity of care by reviewing care gap reports and patient chart records to determine patients with gaps in care. This team member will work under the direction of the director of quality operations, quality operations manager, quality improvement specialists, quality improvement coordinators, Providers, and clinical support staff to assist in closing patient care gaps. To identify open care gaps the review and understanding of payer reports, ACO reports, and other available reports will be a vital asset. Knowledge of HEDIS measures and guidelines will be needed to identify patients due for care gaps closures including wellness visits, screenings, testing, and medication adherence. Chart reviews, chart preparation, clinic outreach, and patient outreach will be conducted by this team member. They will be committed to the continuous review of quality measures, payer reference materials, clinic EMR templates, and other clinical quality reference materials to be remain up to date with the guidelines and to make sure that the Providers and clinical staff are as well.

Qualifications:
- Knowledge of EMR (electronic medical record) and practice management (scheduling/billing) systems
- Proficiency in navigation and chart note documentation
- Understanding of HEDIS measures, including:
- Screening ages for breast and colon cancer
- Exclusions for screenings
- Frequency of screenings
- Familiarity with general clinic workflows and preventative screening processes
- Ability to review Care Gap reports to identify patients due for preventative screenings (e.g., breast and colon cancer screenings)
- Ability to discuss preventative screenings with patients, Providers, and clinical staff
- Tracking progress of care gap closures, patient outreach, and charts reviewed
- Strong communication skills
- Ability to influence or motivate changed behaviors

- Ability to perform any and all assigned duties
- 2 years clinical experience, primary care office preferred
Education:
- High School Diploma or GED
- Medical Assistant or Certified Nursing Assistant
- Other medical certifications will be considered
