HUMAN SERVICES RESOURCES and LINK ACCESS COORDINATOR Revised: 10/04/2023

Position Description Non-Exempt

OVERALL OBJECTIVE OF JOB

This position is the promoter, advocate, and first contact of the Human Services Administration Division for Franklin County. The staff person will be the primary contact for all Franklin County Information and Referral requests for departmental services; as well as the direct connection for the Contact PA 211 program which covers Franklin County.

A connected and coordinated role for this position is to carry out all necessary and related duties as the LINK Services Access Coordinator. The following departments and services are within the Human Services Division: Area Aging Agency; Block Grant funding; Children and Youth Services; Drug and Alcohol; Housing Assistance Programs (HAP); Human Services Development Funds; Information and Referral Services; LINK coordination; Mental Health, Intellectual Developmental Disabilities, Early Intervention, and Mental Health Housing; Tuscarora Managed Care Alliance for behavioral health medical access programs; and Veterans Affairs.

The LINK Services (ADRC) coordination responsibilities are to create a seamless approach in the way assistance and resource referrals are given to community consumers, (ages 59 and younger), living with disabilities. These individuals are in need of accessing and connecting to social determinants of health requirements for healthy living, physical, mental, and behavioral. The ADRC is better known as the Aging and Disability Resource Center within PA.

ESSENTIAL FUNCTIONS OF JOB

1. Coordinate Human Services programs and systems that allow consumers to have access to services and learn ways to access those resources.

2. Responsible for efficient, courteous and knowledgeable customer service to: the Franklin County Resources Information phone line, email address, and walk-in direct contact requests for services, information, or assistance. This also encompasses the requests that rise to the intensive level of LINK assistance for specially identified individuals.

3. Work directly in conjunction with other internal and external community partners to serve as the direct link and bridge for those individuals who are seeking assistance with housing and other assisted daily living-needs.

4. Plans, coordinates, and assists in facilitation of the “Intro to Human Services” Training Day which occurs two times per year; spring and fall. The HS Administrator, Administrative Assistant, and the Program Specialist/Ombudsman personnel are the coordinating staff personnel who assist with this task.

5. Researches and identifies potential community referral sources for the general public and for Human Services professionals.

6. Establishes and maintains a current, comprehensive database of services available to Franklin County residents; this is accomplished through various data collection formats.

7. Works directly with the County GIS and Communication staff to ensure all community connection resource information is updated and current on the County Website and Human Services Administration page.

8. Collects and maintains current and updated forms, information, handouts and brochures of services available to Franklin County residents.

9. Complete all monthly, quarterly and annual reports pertaining to the program.

10. Direct liaison for other human service agencies, partners, and sister departments – responsible for training, information dissemination, and statistics sharing as related to requests for social determinants of health requests.

11. Accompanies the Human Services Administrator or other HS department staff to scheduled presentations, community events, or informational seminars where the Human Services Administration division will highlight and educate the community on programs and services. Independent presentations and public speaking will occur by this position.

12. Develops and administers PA LINK policies and procedures with guidance from the Human Services Administrator and the Area Agency on Aging Director to ensure all components of the program are being established and maintained.

13. Duties related to Franklin County LINK Coordinator as the liaison and advocate for those with verified and self-disclosed disabilities or special consideration functioning needs (individuals aged 59 years and younger):

A. Assist the Area Aging Agency department to perform LINK advocacy and assistance services for any Aging (60 years and older) individuals.

B. Identify and maintain a robust and active LINK network of collaborative partners and community organizations that specifically provide assistance to those with learning barriers, disability differences, or special needs for individualized assistance.

C. Coordinate and conduct LINK partner meetings on a quarterly basis (once every four months). These meetings may be in person or virtual, or a combination of both. Regular communication through telephone calls, emails, or online calendars/newsletters should occur as often as possible.

D. Organize and sponsor LINK cross-training sessions in Franklin County on a quarterly basis.

E. Assist in organizing and planning at least one (1) LINK special-project annually for Franklin County; can be in conjunction with the Aging department or other human services programs. A collaborative event may be held with other service area 9 Counties.

F. Participate in outreach and advocacy activities on behalf of LINK; assist with marketing and outreach of Person-Centered Counseling (PCC) services in Franklin County.

G. Prepare and submit all reports to Cumberland County for LINK (ARDC) and PCC activities in a timely manner as required.

H. Participate in quarterly meetings of local LINK coordinators and other meetings as necessary.

I. Develop positive and amicable relations, educate as many groups and individuals as possible, and distribute LINK/PCC literature through brochures, pamphlets, and other materials.

2. Serves on various state, regional, and local committees as directed.

3. Performs other duties as assigned.

This position receives regular supervision, oversight, and mentoring from the Human Services Administrator.

WORKING CONDITIONS

1. Works indoors in adequate work space, temperatures, ventilation and lighting

2. Works with average indoor exposure to noise and stress, but subject to frequent

disruptions.

3. Normal indoor exposure to dust/dirt.

4. Travels routinely, as needed, to perform essential functions of job.

PHYSICAL/MENTAL CONDITIONS

1. Must possess the intrinsic ability to record, and speak with knowledge and passion while presenting information; have the ability to understand and explain procedures and instructions to all individuals; and be able to communicate fluently and clearly through speech, email, text, video conferencing and public presentations.

2. Must be able to sit for long periods throughout the workday, with intermittent periods of standing, walking, bending, twisting and reaching, as necessary, to carry out job duties.

3. Dexterity requirements range from simple to coordinated movements of fingers/hands; feet/legs; torso, as necessary, to carry out job duties.

4. Sedentary work, with occasional lifting/carrying of objects with a maximum weight of ten pounds.

5. Must be able to pay close attention to details and concentrate on work.

5. Must possess the ability to function independently, have flexibility and the ability to work effectively with diverse clients, coworkers and others.

6. Must possess the ability to maintain confidentiality and understand HIPAA in regard to client personal information and records.

7. Must possess the technical knowledge to proficiently operate a personal computer, including word processing, database and Internet, and be able to operate other office equipment, as needed.

8. Must be able to prepare group presentations, PowerPoint proficiency, and present clear and concise public speaking skills.

9. Must possess a valid driver’s license and a willingness to travel as needed to carry out essential job duties.

10. Must possess the knowledge and ability to maintain accurate records and reports in accordance with established policies and procedures.
