About the Role:
As a Supports Coordinator - Case Manager, you’ll walk alongside individuals with Intellectual and Developmental Disabilities (IDD), including those with Autism, helping them lead self-directed, fulfilling lives. This isn’t time-limited case management — it’s a long-term relationship where you become a consistent advocate, guide, and resource.

Who You Are:
You’re passionate, empathetic, and committed to helping others thrive. You thrive independently, collaborate effectively, and believe in the power of relationships. You understand that case management is about people, not just paperwork.
What You Need:
- A bachelor’s degree with at least 12 credits in social sciences (e.g., sociology, social work, psychology, criminal justice)
OR - 12 social science credits plus 2 years of relevant experience (e.g., developmental disabilities, mental health, special education)
- A valid driver’s license, auto insurance, and a clean driving record
- Ability to pass background checks (Act 33, 34, FBI) and drug screening
- Tech comfort — you’ll be using virtual tools and documentation systems regularly
- Ability to lift up to 15 lbs and travel within the assigned region (rain or shine!)
