Job Opportunity: Supports Coordinator – Intellectual and Developmental Disability Program

Location: 1100 S. Cameron Street Harrisburg, PA 17104

Job Type: Full-time

Pay: $40,000 - $49,000 per year

Expected Hours: 37.5 per week

Schedule: Monday to Friday, Day Shift, No Weekends

What You'll Do:

As a Supports Coordinator, you will:

• Facilitate the creation of person-centered support plans with individuals, families, and support teams.
• Identify and refer community and system supports and services to meet individual needs.
• Coordinate and monitor the delivery of supports and services as outlined in support plans.
• Ensure the quality, type, duration, and frequency of services meet individual needs.
• Monitor health and wellbeing of participants through regular contact with individuals, families, and service providers.
• Identify unmet needs and document them for future planning and funding opportunities.
• Inform individuals of their rights and maintain comprehensive documentation through a statewide web-based system.
• Participate in ongoing training to enhance your skills and knowledge.

Qualifications:

• Bachelor’s degree with at least 12 college credits in sociology, social work, psychology, gerontology, criminal justice, or related social science; OR
• Associate degree with at least 12 college credits in the above fields and two years of relevant experience.
• Required clearances: PA Child Abuse, PA Criminal History, and FBI.
• Valid driver’s license and reliable vehicle.
