About the Role
Fort Hudson Health System is seeking a visionary and clinically-driven Director of Home and Community Services. This is a pivotal Senior Leadership position responsible for steering the strategy, growth, and clinical excellence of our expansive home care program.
The ideal candidate is an experienced Registered Nurse who combines high clinical standards with the creative energy needed to lead a growing program toward long-term success. You will oversee our Licensed Home Care and Adult Day Program operations, ensuring that the care delivered in our community is as exceptional as the care within our walls.
Key Responsibilities
- Strategic Leadership:Provide executive oversight for home care services, ensuring program growth and operational sustainability.
- Clinical Excellence: Maintain and elevate high clinical standards across all home care programs, ensuring compliance with state and federal regulations.
- Team Development: Supervise and mentor a diverse team of healthcare professionals; lead the recruitment, training, and professional development of our Personal Care Assistants (PCAs).
- Innovation: Bring creative ideas to the table to solve long-term industry challenges and improve the quality of life for our residents at home.
- Collaborative Care: Work closely with the Senior Leadership team to integrate community services with the broader Fort Hudson Health System mission.
Required Qualifications
- Education: Valid Registered Nurse (RN) NY license required; BSN preferred.
- Experience: Minimum of 5 years of experience in Home Care (LHSCA or CHHA) or other community-based setting.
- Leadership: Demonstrated experience in employee supervision and team management.
- Communication: Exceptional verbal and written communication skills with the ability to engage stakeholders at all levels.
- Traits: A creative, engaging personality with a "problem-solver" mindset.
Preferred Skills
- Experience with NHTD (Nursing Home Transition and Diversion) and TBI (Traumatic Brain Injury) Waiver programs is highly desired.
- Proven track record in PCA training and curriculum development.
