About the Role:
We’re seeking a dedicated and compassionate Medical Liaison to join our Residential Services team. This vital role ensures the coordination of health services and supports for individuals with intellectual and developmental disabilities (I/DD) living in a Favarh residential home. The Medical Liaison plays a key role in maintaining health records, coordinating appointments, and ensuring medications are administered safely and accurately.

Key Responsibilities:
Medical Coordination & Support:
Schedule and attend medical appointments for residents, including primary care, dental, and psychiatric care
Maintain accurate and up-to-date medical records in compliance with DDS licensing regulations
Communicate with physicians and follow up on all medical recommendations
Ensure completion of paperwork and documentation for appointments and follow-ups
Dispense and/or oversee administration of medications and fill pill boxes weekly
Place medication orders and review Kardex for accuracy
Maintain adequate medical and first aid supplies
Monitor and implement health tracking in Therap with Residential Manager and Nurse
Assist with completing nursing logs and transferring documentation to files
Resident Support & Engagement:
Assist residents with activities of daily living, including hygiene, cooking, dressing, and housekeeping
Support implementation of individual teaching and behavioral programs
Participate in the development and documentation of habilitation goals and IPs
Offer immediate support for minor behavioral issues and escalate more serious concerns
Lead leisure and recreational activities and encourage resident engagement
Promote dignity and civil rights of all residents
Team Collaboration & Training:
Provide on-the-floor training to Residential Instructors as assigned
Communicate effectively with team members and provide shift updates
Serve as backup support to the on-call Manager when needed
Attend and contribute to staff meetings, trainings, and agency events
Administrative & Operational Duties:
Ensure proper documentation in Therap (T-logs, medication logs, behavior tracking)
Report maintenance needs and support budget tracking for participant expenses
Assist residents with banking, budgeting, and personal purchases
Maintain interdepartmental communication and public relations
Minimum Qualifications:
One year of experience in Health, Education, Rehabilitation, or Residential Services
Medication Administration Certification (or willingness to obtain within 3 months of hire)
Must complete required training within 30 days of hire
Physical restraint and CPR training (required within 6 months of hire)
Strong written and verbal communication skills
Ability to work independently and as part of a team
Required Knowledge, Skills & Abilities:
Proficiency in medical coordination, documentation, and DDS regulations
Excellent oral and written communication
Sound judgment and decision-making under pressure
Flexibility, dependability, and attention to detail
Ability to lift up to 75 lbs. and perform physical tasks (CPR, restraints) as needed
Computer literacy preferred (Therap experience a plus)
Physical & Work Environment Requirements:
Frequent standing, walking, sitting, bending, and reaching
Regular lifting of 20–75 lbs.
Use of hands for fine motor tasks and physical assistance
Visual acuity for close and distance vision, depth perception, and focus adjustment
Moderate noise level
