Job Title: RN/LPN MDS Coordinator

Facility: Yamato Nursing and Rehabilitation Center

Location: Boca Raton, Florida

Shift: Full-Time | 8:00 AM – 4:30 PM

Pay Rate: Up to $46 per hour

Job Description

Yamato Nursing and Rehabilitation Center is seeking a knowledgeable and detail-oriented RN/LPN MDS Coordinator to oversee and manage the Resident Assessment Instrument (RAI) process. The ideal candidate will have strong experience with PDPM and Medicare documentation, ensuring compliance with all state and federal regulations while promoting high-quality resident care.

Key Responsibilities

• Accurately complete and coordinate all MDS assessments in compliance with state and federal regulations
• Develop, implement, and maintain individualized care plans based on assessment outcomes
• Collaborate with interdisciplinary teams to ensure continuity and quality of care
• Monitor PDPM requirements and ensure documentation supports accurate Medicare billing
• Review and analyze QI/QM reports and implement corrective actions as needed
• Participate in surveys, audits, and regulatory inspections
• Submit assessments electronically and maintain organized records and schedules
• Educate and support staff on MDS/PDPM updates, processes, and best practices
  
  
  

Requirements

• Active RN or LPN license in the state of Florida
• Proven experience with PDPM and Medicare documentation required
• Minimum of 2 years of experience in a skilled nursing facility preferred
• Strong knowledge of MDS 3.0, care planning, and RAI processes
• Excellent communication, leadership, and organizational skills
