Key Responsibilities:
- Accurately complete and coordinate all MDS assessments in compliance with regulations.
- Develop and maintain individualized care plans based on assessment outcomes.
- Collaborate with interdisciplinary teams to ensure continuity and quality of care.
- Monitor PDPM requirements and ensure documentation supports Medicare claims.
- Review and analyze QI/QM reports, implementing corrective actions when necessary.
- Participate in surveys, audits, and inspections.
- Submit assessments electronically and maintain organized records and schedules.
- Educate staff on MDS/PDPM updates and best practices.
- Active RN license in the state of Florida.
- Proven experience with PDPM and Medicare documentation.
- Minimum of 2 years’ experience in a skilled nursing facility preferred.
- Strong knowledge of MDS 3.0, care planning, and RAI processes.
- Excellent communication, leadership, and organizational skills.
