Key Responsibilities
- Oversee and optimize all aspects of the admissions process to ensure operational excellence and efficiency
- Develop and execute strategic initiatives to maintain and grow facility census
- Build and nurture strong referral relationships with hospitals, physicians, case managers, and community organizations
- Represent the facility at community events to enhance visibility and referral opportunities
- Ensure a patient-centered admissions experience, supporting residents and families with clear communication and compassion
- Verify financial and insurance information prior to admission, ensuring accuracy and compliance
- Collaborate closely with interdisciplinary teams to facilitate smooth transitions of care
- Maintain up-to-date knowledge of Medicare, Medicaid, Managed Care, CMS guidelines, and Department of Health regulations
- Track, analyze, and present monthly admissions data and trends to leadership
- Lead, mentor, and support admissions staff to ensure a high-performing and cohesive team
- 3–5 years of experience in admissions, intake, case management, or healthcare business development
- Prior experience in a skilled nursing facility (SNF) or post-acute care setting strongly preferred
- Director-level experience in a skilled nursing environment required
- Strong understanding of Medicare, Medicaid, Managed Care, and payer authorization processes
- Experience working collaboratively with interdisciplinary clinical teams
- Knowledge of state and federal regulations related to admissions and transfers
- Proficiency with EMR systems and referral platforms, including: PointClickCare, CarePort, NaviHealth, AllScripts
- Experience with insurance verification and authorization platforms such as: Navinet, CarePort, Availity, Waystar, ePACES, Connex
