Key Responsibilities
- Evaluate and monitor factors impacting admissions; identify trends and assist in implementing solutions
- Support marketing and outreach initiatives by attending community events and cultivating referral relationships
- Serve as a liaison between the facility, hospitals, and referral sources to coordinate seamless admissions
- Communicate facility policies, visitor guidelines, and care procedures clearly to residents and families
- Verify insurance coverage and financial information prior to admission and collaborate with the interdisciplinary team
- Maintain strong relationships with hospitals, physicians, case managers, and community partners
- Ensure timely and accurate completion of all admission documentation
- Stay current with Medicare, Medicaid, Managed Care, and reimbursement guidelines
- Track, analyze, and report monthly admissions data and trends
- Minimum of 2 years of admissions experience in a Skilled Nursing Facility (SNF), Assisted Living Facility (ALF), or healthcare setting
- Strong knowledge of Medicare, Medicaid, Managed Care, and reimbursement models (including PDPM and HMO guidelines)
- Familiarity with EMR systems
- Understanding of CMS regulations and Department of Health (DOH) guidelines
- Excellent interpersonal, communication, and organizational skills
- Ability to multitask and work effectively in a fast-paced environment
