- Performing home visits to conduct functional, psychosocial and environmental safety assessments
- Determining eligibility of enrollees
- Establishing an individual plan of care that enables them to remain safely at home despite long-term care needs
- Ensuring that DOH UAS information is complete and accurate
- Conducting follow-up assessments to ensure the members are receiving the proper care and services.
- Performing other duties, as needed
- NYS Registered Nurse (RN) license
- Prior UAS, MLTC or Home Care experience
- Bachelor's Degree in Nursing
- Bilingual
Salary : $90,000 - $110,000
