A healthcare network in New York City is currently seeking a new, licensed Registered Nurse (RN) to join their team in the Bronx as their new Hospice Field Case Manager. In this role, the Hospice Field Case Manager (RN) will be responsible for visiting patients in both the community and facility settings to perform both physical and psychosocial assessments and electronically document visit occurences.

- Develop and implement nursing plans of care
- Make recommendations for care based on patients' needs
- Contact the patient's Physician to request written orders to facilitate plan of care change
- Supervise, train and evaluate Home Health Aides and homemakers working with our hospice patients
- Perform other duties, as needed
- Associate's Degree in Nursing
- NYS Registered Nurse (RN) license
- Solid assessment, clinical, and documentation skills
- Patient oriented
- Great interpersonal skills
- Excellent communication skills (written and verbal)
- Strong attention to detail
- Highly organized
- 2 years of experience in a Medical-Surgical, Community Health, and/or Oncology Nursing setting
- Bachelor's Degree in Nursing
Salary : $100,000 - $119,000
