Job Description:
- Interview clients to determine and verify program eligibility, competencies, and needs.
- Conduct outreach to identify eligible families and support program objectives.
- Develop and maintain Individual Service Plans and/or Residential Service Plans.
- Identify client barriers and coordinate access to appropriate services and resources.
- Gather and document medical, psychological, and social information relevant to each case.
- Refer clients to appropriate government agencies, community resources, and partner organizations.
- Create, update, and maintain accurate client and employer case records and documentation.
- Maintain regular contact with clients through scheduled visits, phone calls, emails, and texts.
- Collect, compile, and report data as required.
- Provide in-person, over-the-counter, and phone assistance, including eligibility guidance and program information.
- May operate a company vehicle for business purposes.
- Valid driver’s license with a clean driving record required.
- Working knowledge of computer applications with basic typing skills, data entry, and standard office equipment.
- Understanding of housing assistance and placement programs, social service agencies, and community resources.
- Ability to work effectively with individuals from diverse cultural, social, and economic backgrounds.
- Strong interviewing, motivational, and public relations skills.
- Ability to build and maintain effective working relationships with community agencies and partners.
- Strong organizational and time-management skills with the ability to prioritize multiple projects and meet deadlines.
- Collaborative team player with a strong desire to help others.
