Job Function~
- Day Shift.
- 9am - 5pm.
- Participates in the training of members of the patient care team.
- Evaluates performance of the Medical Assistant and Registered Nurse.
- Coordinates requests for medication refills according to Housing Works procedures and protocols.
- Coordinates outpatient work-up and/or treatment plans when the patient situation requires it (e.g., further clinical testing, x-rays) and facilitates communication between physicians and health care providers.
- Utilizes EMR to document all patient care and medication administration accurately and in a timely manner, in compliance with PSI policies and procedures.
- Bachelor’s Degree or Associates Degree in Nursing from an accredited nursing program.
- Current registration and active license as a New York State Registered Nurse.
- CPR certification or completion of certification during employment.
- Demonstrated understanding of the needs of clients in treatment for HIV/AIDS and substance abuse.
- QHC Management experience preferred.
