Key Duties & Responsibilities
- Conduct intake assessments and personal interviews to gather client developmental, medical, and background information.
- Obtain client consent for information disclosure during review meetings.
- Provide follow-up services and conduct onsite reviews to ensure continuity of care and program compliance.
- Maintain accurate client case files; document all RECA claim progress, DOJ reviews, and case notes.
- Schedule and conduct home visits to assist with RECA claim forms.
- Ensure confidential information is securely handled (storage, digital conversion, retention/destruction).
- Assist clients in obtaining necessary documentation (IHS RESEP results, vital records, proof of residency, employment, Social Security, etc.).
- Coordinate activities with Navajo Nation, State/Federal agencies, schools, Head Start programs, Navajo Chapters, and community-based programs.
- Educate clients in self-sufficiency, nutrition, health, financial management, and home care.
- Represent the program at conferences, seminars, workshops, staff meetings, and case staffing sessions.
- Prepare and submit weekly, monthly, and quarterly reports on RECA claims and client visits.
- Perform other duties as assigned to support program goals and Navajo Nation objectives.
- High school diploma/GED; and 1 year experience in social services/human services; or equivalent combination of education and experience
- Possess Valid state driver’s license
- Knowledge of tribal, federal, state, and local laws, ordinances, and regulations
- Principles and practices of social service work and human service development
- Navajo Nation regulations regarding client care
- Community resources available to individuals and families
- Computer literacy and software proficiency
- Oral and written communication skills in both Navajo and English
- Interpersonal communication skills and relationship-building
- Analytical and assessment skills; ability to provide appropriate remedies
- Workload management and judgment in safeguarding confidential information
- Extensive travel required
