Duties
- Perform comprehensive patient assessments including vital signs, physical examinations, and health histories to inform care plans.
- Administer medications, injections, IV infusions, and other treatments following strict protocols and safety standards.
- Document patient information accurately using Electronic Health Records (EHR) systems such as Epic, Cerner, or Athenahealth.
- Assist with diagnostic evaluations, specimen collection, and processing to support accurate diagnoses.
- Provide specialized care for patients with complex needs such as dementia, Alzheimer's disease, or those requiring hospice or palliative care.
- Manage airway procedures, catheterizations, tube feedings, and ventilator support as needed for critical or long-term care patients.
- Collaborate with interdisciplinary teams in settings like skilled nursing facilities, hospitals—including Level I Trauma Centers—and outpatient clinics to ensure seamless patient care.
Requirements
- Valid LPN license in good standing with the state licensing board.
- Proven experience working in diverse healthcare environments such as hospitals, nursing homes, assisted living facilities, or home care.
- Strong knowledge of medical terminology, anatomy, physiology, and infection control practices.
- Experience with EMR/EHR systems including Epic, Cerner, or eClinicalWorks; proficiency in medical documentation is essential.
- Competency in performing IV insertions, venipuncture (phlebotomy), medication administration, and basic life support (BLS).
- Familiarity with long-term care regulations and geriatric patient needs; experience working with individuals with disabilities or developmental delays is a plus.
- Ability to communicate effectively with patients and caregivers while maintaining confidentiality according to HIPAA policies.
