Position Summary:
The MDS Coordinator is responsible for coordinating and completing the Minimum Data Set (MDS) assessments in accordance with federal and state regulations. This role ensures accurate and timely resident assessments, care planning, and reimbursement optimization while supporting quality resident outcomes in a long-term care setting.
Key Responsibilities:
MDS & Care Planning
- Complete and submit accurate and timely MDS assessments (admission, quarterly, annual, significant change, discharge)
- Coordinate interdisciplinary care plan meetings
- Ensure comprehensive, individualized care plans are developed and updated
- Maintain compliance with RAI Manual guidelines
- Ensure compliance with CMS, Medicare, Medicaid, and state regulations
- Maintain survey readiness related to MDS accuracy and documentation
- Assist with audits, surveys, and Plans of Correction (POC)
- Optimize reimbursement through accurate coding and documentation
- Monitor PDPM components and case mix index (CMI)
- Collaborate with therapy, nursing, and billing teams
- Review medical records to ensure documentation supports MDS coding
- Educate staff on documentation requirements
- Identify and correct discrepancies in clinical records
- Work closely with nursing, therapy, dietary, and social services
- Communicate resident status changes impacting assessments
- Participate in care conferences and QAPI meetings
- Monitor quality measures and identify trends
- Participate in performance improvement initiatives
- Assist in reducing survey risk related to documentation and assessments
- Current RN or LPN license in Tennessee.
- MDS experience in long-term care preferred
- Strong knowledge of RAI Manual and PDPM
- Experience with electronic medical records (EMR) systems
- Strong organizational and time management skills
- MDS Certification (RAC-CT or similar)
- Experience with Medicare/Medicaid reimbursement systems
- Prior experience in skilled nursing facility (SNF) setting
- Strong attention to detail and accuracy
- Ability to meet strict deadlines
- Knowledge of regulatory requirements and survey process
- Effective communication and teamwork skills
- Analytical and problem-solving abilities
- Ability to sit for extended periods (documentation work)
