- Conduct comprehensive patient assessments, including vital signs, medical history review, and physiological evaluations to determine admission suitability.
- Coordinate with interdisciplinary teams to facilitate seamless intake processes and develop individualized care plans aligned with each resident’s needs.
- Manage medical records accurately, ensuring compliance with HIPAA regulations and proper documentation of all admission activities.
- Perform diagnostic evaluations and initial case management to establish baseline health status for new residents.
- Assist with discharge planning by collaborating with families, physicians, and external providers to ensure continuity of care post-admission.
- Monitor infection control protocols and ensure adherence to hospital medicine standards during the admission process.
- Utilize critical care knowledge and hospital experience to handle complex cases involving geriatrics, behavioral health, or post-acute care needs.
- Valid RN or LPN license with current state licensure required; experience in a hospital or inpatient setting preferred.
- Demonstrated expertise in patient assessment, vital signs monitoring, and medical terminology comprehension.
- Prior experience in skilled nursing facilities, post-acute care, or outpatient clinics is highly desirable.
- Knowledge of HIPAA regulations, discharge planning procedures, and medical records management essential.
- Experience with diagnostic evaluation, utilization management, case management, and intake processes preferred.
- Strong communication skills to effectively collaborate with healthcare teams, residents, and families.
