What You'll Do
- Provide skilled hospice nursing care in patients' homes and residential settings.
- Complete comprehensive nursing assessments and develop individualized plans of care.
- Manage pain and symptoms while promoting comfort and dignity.
- Educate and support patients, families, and caregivers throughout the hospice journey.
- Collaborate with physicians, social workers, chaplains, CNAs, and the interdisciplinary team.
- Complete timely, accurate electronic documentation.
- Participate in shared on-call coverage as needed.
What You'll Bring
- Current Arizona Registered Nurse (RN) license.
- Minimum one year of RN experience (hospice or home health experience preferred).
- Associate's or Bachelor's degree in Nursing.
- Current CPR certification.
- Valid driver's license, reliable transportation, and current auto insurance.
- Strong clinical judgment, excellent communication skills, and a compassionate, patient-centered approach.
- Ability to work independently while collaborating with a dedicated hospice team.
