- Collaborate closely with physicians, social workers, therapists, and other healthcare professionals to optimize patient outcomes.
- Oversee discharge planning and ensure seamless transitions between care levels while maintaining compliance with health regulation policies.
- Utilize Electronic Health Records (EHR) systems to document patient information accurately, manage medical records, and track clinical progress.
- Provide education to patients and families about hospice services, symptom management, and advanced directives to promote informed decision-making.
- Engage in case management activities such as medication reconciliation, vital signs monitoring, diagnostic evaluation, and managing complex patient needs.
Requirements
- Valid nursing license with current registration; experience in critical care settings such as ICU, emergency room, or trauma centers highly preferred.
- Demonstrated expertise in hospital medicine, medical-surgical units, or post-acute care environments; experience with hospice or palliative care is a plus.
- Strong knowledge of medical terminology, physiology, HIPAA regulations, and health policy compliance.
- Proven ability to perform comprehensive patient assessments and manage complex clinical cases effectively.
- Proficiency in Electronic Health Records (EHR) or Electronic Medical Records (EMR) systems for documentation and data management.
