- Provide compassionate personal care to hospice patients in their homes and care facilities.
- Assist with activities of daily living, including bathing, dressing, grooming, toileting, and mobility.
- Observe and report changes in patients' physical or emotional condition to the hospice nurse.
- Offer comfort, companionship, and emotional support to patients and their families.
- Document patient care accurately and promptly.
- Work collaboratively with the interdisciplinary hospice team to ensure exceptional patient care.
- Current Arizona Certified Nursing Assistant (CNA) license.
- Minimum of one year of CNA experience required.
- Hospice or home health experience preferred.
- Current CPR certification.
- Valid Arizona driver's license, reliable transportation, and current auto insurance.
- Excellent communication skills and a compassionate, patient-centered approach to care.
