Your Responsibilities
Conduct in-home patient assessments and develop individualized care plans
Administer medications, wound care, and treatments as prescribed
Educate patients and families on disease management, safety, and independence
Monitor patient progress and adjust care plans as needed
Supervise LPNs and Home Health Aides to ensure quality patient care
Collaborate with an interdisciplinary team to coordinate comprehensive patient care
Qualifications
Active Arizona Registered Nurse (RN) license in good standing
Minimum of one year of nursing experience preferred
Home health, hospice, rehab, or critical care experience is a plus
New graduates encouraged to apply
Current CPR certification
Valid driver’s license and reliable transportation
Strong communication, organizational, and interpersonal skills
