Key Responsibilities:
- Plan, organize, and provide personalized home care services
- Conduct thorough patient assessments and develop customized care plans
- Collaborate with care teams and communicate patient conditions to primary physicians
- Educate patients and families on healthcare management and wellness
- Maintain accurate clinical notes and ensure compliance with care standards
What We’re Looking For:
- Licensure: Valid RN license from the Arizona State Board of Nursing
- Experience: Minimum one year in home health or hospice preferred
- Transportation: Valid Arizona driver’s license and reliable vehicle
- Certifications: Current CPR certification & negative TB test (assistance available)
- Background Clearance: Satisfactory fingerprint clearance per federal/state regulations
- Bilingual Preferred: Conversational Spanish skills highly desired
Education Requirements:
- Associate’s Degree in Nursing or higher
