Key Responsibilities
- Assess and evaluate patients' therapeutic, rehabilitative, and functional needs.
- Develop and implement individualized care plans tailored to each patient’s condition.
- Evaluate home environments for safety and recommend modifications as needed.
- Provide direct physical therapy treatments and educate family members or caregivers.
- Identify additional home health services needed and make appropriate referrals.
- Prepare and submit clinical progress reports based on goal achievement.
- Offer in-service education for nursing staff as necessary.
- Supervise Physical Therapy Assistants in accordance with policies and state regulations.
Qualifications
- Education: Bachelor's degree in Physical Therapy or higher from an accredited institution.
- Licensure: Active Physical Therapist license in Arizona.
- Experience: At least one year of relevant experience; community/home health experience preferred.
- Language Skills: Bilingual (English/Spanish) strongly preferred.
- Skills: Strong verbal and written communication skills with excellent organizational abilities.
- Certifications: Current CPR certification.
