In this rewarding role, you’ll provide personalized in-home care that supports the physical, emotional, and spiritual well-being of each patient. You’ll work alongside an interdisciplinary healthcare team to help patients achieve the highest quality of life while remaining safely and comfortably at home.

Key Responsibilities
Conduct in-home patient assessments and develop individualized care plans
Administer medications, treatments, and wound care per physician orders
Educate patients and families on disease management, safety, and wellness strategies
Monitor patient progress and adjust care plans as needed
Supervise and document care provided by LPNs and Home Health Aides (HHAs)
Participate in interdisciplinary team meetings to coordinate high-quality patient care
Qualifications
Current Arizona RN license in good standing
Minimum of one year RN experience preferred
Home health, hospice, rehab, or critical care experience is a plus, but not required
New Grad Nurses encouraged to apply through our Ambassador Program
Current CPR certification
Valid driver’s license and reliable transportation
Strong communication, organizational, and interpersonal skills
