Your Role
- Perform in-home patient assessments and create personalized care plans.
- Administer medications, wound care, and treatments per physician orders.
- Educate patients and caregivers on condition management and independence.
- Monitor and document patient progress, reporting updates to the care team.
- Supervise LPNs and Home Health Aides to ensure quality and compliance.
- Participate in interdisciplinary meetings to coordinate holistic care.
- Active Registered Nurse (RN) license in Arizona.
- Minimum of one year of nursing experience (home health, hospice, or critical care preferred).
- CPR certification and valid driver’s license with reliable transportation.
- Excellent clinical judgment, communication, and time-management skills.
