Your Role:
Provide skilled nursing care to patients in their homes.
Administer medications, treatments, and wound care as directed.
Monitor patient conditions and report changes to the supervising RN.
Support patient education and promote independence in daily care.
Collaborate with the care team to deliver high-quality, coordinated services.
What You Need:
Active LPN license in the State of Arizona.
CPR certification and valid driver’s license with reliable transportation.
Excellent communication, organization, and patient care skills.
Home health experience a plus, but not required — we’ll train the right person!
