Qualifications:
Minimum Education, Experience and Training: Six months of experience as a Human Services Case Manager, or County Human Services Intern; or one year of experience in public or private social work and a Bachelor's Degree which includes or is supplemented by 12 semester hours credit in sociology, social welfare, psychology, gerontology, or other related social sciences; or a bachelor's degree with a social welfare major; or any equivalent combination of experience and training including successful completion of 12 semester hours credit in sociology, social welfare, psychology, gerontology, or other related social sciences.

Essential Job Functions:
CARE MANAGEMENT PROGRAMS (OPTIONS, CSP, GRANDPARENT)
- Carries out all duties as required by the Pennsylvania Department of Aging Policy and Procedure Manual Chapter for Caregiver Support Program funded services, the Care Management Chapter and as needed the OPTIONS Chapter.
- Complete required program assessment tools (CAT, NAT, or NAT-E) to assess an individual’s medical, financial, functional, social and psychological status for program appropriateness and eligibility and caregiver needs. Make recommendations and develop service/care plans and/or provide resources based on assessment outcome which meet the needs of the individual; monitor individual’s status and recommend appropriate changes accordingly.
- Authorize and coordinate the delivery of services with individuals, providers of choice and informal support.
- Complete required monitoring contacts to ensure care plan and services continue to meet individual’s/caregivers’ needs. Make changes as needed and/or appropriate.
- Accurately document and maintain assessments, reassessments, and individual contacts in case notes and journal entries; prepare required activity reports as necessary.
- Review and process receipts in accordance with Caregiver Support Program regulation and policy.
- Organizes the Caregiver Support and Grandparenting Support Groups as appropriate.
- Oversee and train CSP volunteers.
- Implement and coordinate specialty program(s) as assigned.
- Maintain required clearances and education, including Child and State, and Federal if needed.
- Complete the Assessment (Level 1 and Level 2) process in accordance with regulation and Aging Well directives, contract, and procedures.
- Assessments may be completed in Nursing Facilities, Personal Care Homes, the community/consumers’ homes, hospitals, correctional facilities for placement, Home and Community based services through OPTIONS or waiver/CHC, LIFE program, SSI, or MA.
- Be a credentialed assessor through Aging Well for FED Assessments and maintain credentials. Complete training in the Pennsylvania Individualized Assessment (PIA) system and the Functional Eligibility Determination (FED) assessment.
- Complete Desk Reviews within required timelines and Desk Review process.
- Prepare the documents needed for Medical Review cases.
- Participate in Appeals/Hearings for assessments.
- Process previously assessed files.
- Inform consumers of the appeal process regarding disagreements with the level of care determination.
- Accurately document and maintain assessments, reassessments/Desk Reviews, and individual contacts in PIA or other required systems.
- Complete protective service reports and/or incident reports as required; may be required to participate in the protective services on-call system duty assignments.
- Provide information, assistance, and referral services, including the completion of applications and forms.
- Interpret and/or explain agency programs, federal, state and county policies and procedures for individuals, groups, and agencies.
- Maintain certification for required programs.
- Crosstrain and provide backup coverage for other programs assigned.
- Travel as necessary, including meeting with and assisting individuals in accessing needed support and services.
- Prepare studies and reports as necessary.
- Attend meetings, training, and seminars as necessary.
- Performs other job-related duties as may be assigned.
