**Required for presentation: must have actual state license board certification in hand, and active DEA for the state of application and CSR if needed per state in hand. Must be able to perform ICU procedures and have case logs**

All candidates must cross credential at ProHealth Waukesha Memorial Hospital and ProHealth Oconomowoc Memorial Hospital.

**Days and Nights required**

Start in April/May for 7-10 shifts per month

1 APP at night 1900 to 0700 with MD on-call

NP Requirements Summary

Application Packet (appcentral), copy of government issued Photo ID (DL or Passport), past 2 years case logs in specialty requested, current CV, vaccination Records to include TB (2 skin tests within the past 12 months, one within 90 days of start or Quantiferon blood test done within 90 days of start), Hep B, Flu, Rubella, COVID (no longer required. Please send if received); ACLS and copies of prior malpractice insurance past 5 years.
