What You'll Do
- Complete accurate MDS assessments and documentation
- Develop and update individualized care plans
- Support quality resident care across the interdisciplinary team
- Ensure compliance and proper reimbursement for services
What You Need
- Active Illinois RN or LPN license
- Long-Term Care experience required
- MDS experience preferred (1–2 years)
- Strong understanding of LTC regulations and assessments
