Position Summary
We are seeking a detail-oriented and experienced MDS Coordinator to join our Long-Term Care team. The MDS Coordinator is responsible for coordinating and completing accurate and timely Minimum Data Set (MDS) assessments in compliance with federal and state regulations, ensuring optimal reimbursement and quality resident care.
Key Responsibilities
- Coordinate, complete, and submit MDS assessments (OBRA, PPS, and Medicare) accurately and on time
- Ensure compliance with CMS, state, and facility regulations
- Collaborate with interdisciplinary team members to develop and update individualized care plans
- Monitor RUG/PDPM classifications and ensure appropriate documentation to support reimbursement
- Conduct care plan meetings and participate in Quality Assurance and Performance Improvement (QAPI) activities
- Provide education and guidance to nursing staff regarding documentation and assessment requirements
- Maintain accurate resident records and support survey readiness
Qualifications
- Active RN or LPN license in good standing
- Minimum 1–2 years of MDS experience in a long-term care or skilled nursing facility
- Strong knowledge of MDS 3.0, PDPM, and Medicare/Medicaid regulations
- Excellent organizational, communication, and time-management skills
- Proficiency with electronic medical records (EMR) systems
- RAC-CT or MDS certification preferred (or willingness to obtain)
