MDS Key Responsibilities:
- Oversee and coordinate the Resident Assessment Instrument (RAI) process in full compliance with current regulations and facility guidelines.
- Collaborate with the Interdisciplinary Care Plan Team to develop personalized and comprehensive care plans for each resident.
- Conduct and/or coordinate resident interviews as part of the assessment process.
- Assess residents’ medical conditions and identify needs for specialized evaluations (e.g., Norton Scale, behavior management, psychotropic medication usage).
- Complete accurate, timely preliminary and full assessments, using approved tools and documentation.
- Ensure every section of the RAI is properly signed and dated by the responsible staff member.
- Certify the completion of assessments with your signature and date.
- Provide data and insight to support the facility’s Quality Assurance Program.
