CORA Services is seeking an Attendance Case Manager to support the Community Schools team at George Washington High School (19116 zip code).
The Attendance Case Manager is solely dedicated to one community school (George Washington High School) and is responsible for providing case management services and supports to young people and their families for whom risk factors for truancy have been identified. The ACM will work collaboratively with the Community School Coordinator and Partners to create a positive school culture that celebrates attendance and encourages regular school involvement. Attendance Case Manager works as a member of the Integrated Case Management team, supporting George Washington youth, families and school community to ameliorate barriers to regular school attendance and assist in creating a supportive school environment. The Attendance Case Manager would have an office at George Washing High School and work primarily from the school as an integral support to the attendance team.
STARTING SALARY: $21.40/hour (rate commensurate with experience and credentials)

In addition, as a Attendance Case Manager your duties will include:
- Engage with students/families in a collaborative relationship that will enhance the potential for the client’s completion of service with positive outcomes.
- Collaborate with school to develop and implement school-wide Incentive Plan
- Participate in SDP Attendance Team or Multi-Tiered System of Supports and work closely with schools, Community School Coordinators and other School Partners
- Develop a plan of service for each child assigned to them that could include enrollment in services such as Out-of-School Time and provide them with the information, tools, school supplies, uniforms, and other resources to attend school regularly
- Work on referred cases in collaboration with the school prior to or during the SAIC/SAIP process to remove the barriers to attending school and to slow or stop the accrual of absences, ultimately reducing the number of referrals to Regional Truancy Court
- Conduct regular home visits, as needed to include assessing the families’ needs for services and developing a service plan to remove barriers to regular school attendance.
- Complete weekly contact with families on assigned caseload to support with social services and health navigation as needed.
- As a member of the Integrated Case Management Team, collaborate with school leaders and partners to assess needs of school community and work to plan, coordinate and implement Family Forums and outreach activities to support the young people and families.
- Maintain accurate and timely documentation of services provided to student/family
- Maintain an awareness of community resources and facilitate student/family access to appropriate resources and services necessary to support and supplement service goals.
- Advocate with and for clients for access to appropriate level of care placements.
- Adhere to Agency and regulatory entity guidelines concerning caseload requirements, service delivery, accountability and confidentiality.
- Participate in all necessary interdisciplinary functions and meetings.
- Adhere to training requirements established by Agency or other regulatory entities.
- Attend regular supervision and department meetings.
- Perform other duties as assigned by the Truancy Case Management Supervisor, as needed.
- Bachelor’s (or master’s) degree in social work, human services or related discipline
- 2 years’ work experience in a social services capacity; treatment or intervention services experience preferred
- Working knowledge of delinquency prevention concepts and the effects of substance use and abuse
- Demonstrated case management experience preferably with young people and families
- Outstanding communication and interpersonal skills; ability to establish and maintain positive, productive working relationships internally and with those external to CORA (students, families, teachers, school administration, etc.)
- Valid driver’s license required
- Proficient with Office 365 applications
