Role: Speech-Language Pathologist providing evaluation, treatment, and therapy services in a home setting.
Responsibilities: Conduct assessments, develop individualized treatment plans, implement therapy strategies, and modify interventions based on ongoing evaluations.
Communicate effectively with patients, families, and interdisciplinary team members. Prepare documentation including evaluation results, progress notes, and treatment reports. Participate in program development and supervise clinical internships.
Qualifications: CCC-SLP from ASHA or active supervised clinical fellowship, or active state license. Prefer 1-3 years of experience in hospital, SNF, outpatient, or home health settings.
Skills: Excellent communication, ability to work collaboratively within a team, and adherence to safety and confidentiality standards.
