- Provide home healthcare services to pediatric patients, primarily toddlers, with low acuity and GTube needs.
- Deliver compassionate, patient-centered care in clients' homes, ensuring positive clinical outcomes.
- Utilize electronic documentation via tablets in patient homes.
- Collaborate with a multidisciplinary team to support patient health and safety.
- Maintain accurate records, adhere to safety protocols, and follow care plans.
- Active, unencumbered LPN/LVN license in the practice state; compact licenses must be transferred within 90 days.
- Current CPR certification with hands-on component; assistance available post-hire if needed.
- TB skin test within the past 12 months.
- Reliable transportation required.
- Six months of hands-on nursing experience preferred but not mandatory.
