Provides supervision, leadership, and guidance to staff of HCS, TxHml, GR, PASRR, CFC, ECC, CLASS, Access and Intake, Enrollment, and CLOIP service coordinators. Provides direction under the Service Coordination functions rule in the Texas Administrative Code, 40 TAC, Chapter 2, Subchapter L: Service Coordination for individuals with IDD. Assists with the TST/MFP program. Supervision of service coordinators to assure that the development of each assigned consumer’s Person Directed Plan is individualized, outcome orientated and meets HHSC guidelines. Data entry for in-coming and out-going transfers. Monitors service coordinator’s documentation. Provide monthly and quarterly reports to supervisor and partnering agencies. Assures service coordinators make monthly contacts with each assigned consumer. Resolves issues/concerns that may arise; and ensures established timeframes are adhered to. “Hands-On” approach to supervision is a must. Develop and maintain caseloads for staff. The position will work closely with private and public providers, staff, individuals, partnering agencies, and family members/LAR.
EDUCATION, TRAINING, & EXPERIENCE
Graduation from an accredited four-year college or university with major course work in human services, business, or another related field is required.
ESSENTIAL TASKS
- Two years of related work experience is required.
- Knowledge of IDD programs (provider and authority) preferred.
- Must be organized and remain current on all job responsibilities.
- Skills in organizing complex information.
- Capability of interpreting and teaching others.
- Skills in data analysis and report preparation for review and presentation.
- Ability to clearly communicate complex and sensitive information both orally and in writing.
- Demonstrate the ability to work closely with all levels of service providers in an atmosphere of teamwork.
- Adaptability to change is essential.
- Proficient in usage of SmartCare, CARE, TMHP, multiple HHSC portals, and Microsoft programs.
- Must be able to multi-task.
- Must be able to change direction/focus multiple times a day.
REQUIREMENTS
- Valid Texas Driver License required.
- Must not have more than 3 moving violations in the last 3 years.
- Must be insurable on the Center’s insurance.
- Must be able to pass a background check.
- Must be able to complete all MHMR Concho Valley training.
SHIFT HOURS
Monday through Friday, 8:00 a.m. – 5:00 p.m.
SALARY RANGE
$45,500 - $49,000 Annual/DOE
