RTC Liaison Case Management
JOB SUMMARY: The employee provides case management functions as outlined by the Texas Administrative Code following Resiliency and Disease Management Principles. This includes assessment, treatment planning, monitoring of services, crisis services and the provision of skills training as assigned and specified. Case managers should develop and maintain referral resources for service provision allowing for consumer choice, where available, of providers. This employee works to ensure the best value of resource allocation and quality of service provision within a recovery-based framework. Case managers observe individuals in a variety of situations for background information, monitoring of progress and treatment services. Assist individuals and families to access medical, social, educational, and other appropriate services that will help the child achieve a quality of life and level of care acceptable to the family. Provide Crisis prevention and management services to include location and coordinating services and supports to prevent or manage a crisis.
RTC Liaison Additional Duties: The duties of the continuity of Care Liaison are to assist families that are requesting help to have their child placed in a residential treatment center (RTC) due to their mental and behavioral issues that make them a risk to themselves or others.
- At the initial meeting to discuss the RTC project with families to make sure that they are currently in services and receiving the treatment that is appropriate while in the process of completing paperwork that is required from the RTC Project.
- Send in CRCG referral for the family and attend CRCG meetings to support the family.
- Meet and assist families with all paperwork that is required to submit to the RTC Project.
- Attend RTC Project Quarterly TA Calls.
- Provide ongoing communication/updates to all parties involved, not limited to RTC Staff, Families, Case Workers, and Probation officers) at a minimum of twice a month and document this in the EMR.
- Attend all treatment planning to clients who have been placed in the care of an RTC in order to assist continuity of care.
- Assist the Program Director with Concho Vally Community Resource Coordination Group monthly.
- Other duties as assigned.
EDUCATION REQUIRED:
Bachelor’s degree in a Center approved Human Services field. Bachelor or advanced Degree from an accredited college or university with a major in a social, behavioral, or human service field including psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, and education.
LICENSE/CERTIFICATION REQUIRED:
- Must be insurable on the Center’s insurance.
- Valid Texas Driver’s License required:
- Must not have more than 3 moving violations in the last 3 years, and proof of current liability insurance.
- Must be willing to work towards certification as a Qualified Mental Health Provider (QMHP-CS)
EXPERIENCE REQUIRED: Preference will be given to previous experience working with children/adolescents, families, schools, and the Juvenile Justice System. A minimum of six months or more working with or in the field of mental health/illness or social work experience is preferred.
SHIFT HOURS
Monday through Friday, 8:00 a.m. – 5:00 p.m.
